Evaluation of cognitive schemas based on the presence of anxiety disorder among coronary artery disease patients

Abstract
Aim: The relationship between psychological factors and cardiac diseases has been concerned for a long but our knowledge about the cognitive profiles which could be important etiological factor in cardiac disease and comorbid psychiatric disorders, is limited. In this study, we aimed to compare the cognitive profiles of coronary artery disease patients with or without comorbid anxiety disorder.
Methods: A total of 35 coronary artery disease patients with anxiety disorder and 20 coronary artery disease patients without comorbid anxiety disorders were enrolled into the study. Beck Anxiety Inventory and the Young Schema Questionnaire-Short Form 3 were applied to each participant.
Results: Disconnection, rejection, impaired autonomy self-manifestation, extreme vigilance, unrelenting standards, other-directedness, and impaired limits schema domain scores were significantly higher in coronary artery disease patients with anxiety disorder comorbidity compared to coronary artery disease patients without anxiety disorder .
Conclusion: Our study have revealed significant differences in cognitive profiles of coronary artery disease patients with comorbidity of anxiety disorders. These findings show the importance of cognitive profiles which were observed in coronary artery disease patients with anxiety disorders. Schema-focused approach might be useful in the treatment of such patients. Further studies with schema-focused therapy approaches are needed to illustrate the issue.

Koroner arter hastalarında anksiyete bozukluğunun varlığına göre bilişsel şemaların değerlendirilmesi

Ozet
Amaç: Psikolojik faktörler ve kardiyolojik hastalıklar arasındaki ilişki uzun zamandan beri tartışılmaktadır. Fakat kardiyolojik hastalıklar ile komorbid psikiyatrik hastalıklarda etiyolojik olarak önemli olabilecek bilişsel profiller ile ilgili bilgilerimiz sınırlıdır. Bu Çalışmamızda komorbid anksiyete bozukluğu olan koroner arter hastaları ile komorbid anksiyete bozukluğu olmayan koroner arter hastalarının bilişsel profilleri açısından karşılaştırılması amaçlanmıştır.
Yöntem: Çalışmaya 35 komorbid anksiyete bozukluğu olan koroner arter hastası, 20 komorbid anksiyete bozukluğu olmayan koroner arter hastası dahil edildi. Katılımcıların tamamına beck anksiyete ölçeği ve Young Şema Ölçeği-Kısa Form 3 uygulandı.
Bulgular: Anksiyete bozukluğu komorbiditesi olan koroner arter hastaları ile anksiyete bozukluğu komorbiditesi olmayan koroner arter hastalarına göre kopukluk ve reddedilmişlik, zedelenmiş otonomi ve kendini ortaya koyma, zedelenmiş sınırlar, diğeri yönelimlilik, aşırı tetikte olma ve bastırılmışlık şema alanı puanlarının anlamlı düzeyde yüksek olduğu saptandı.
Sonuç: Çalışmamız, anksiyete bozukluğu komorbiditesi olan koroner arter hastalarında bilişsel profiller anlamlı farklılık olduğunu göstermektedir. Bu bulgular koroner arter hastalığında gözlemlenen anksiyete bozukluklarında bilişsel profillerin önemini göstermektedir. Bu hastaların tedavisinde şema odaklı yaklaşım faydalı olabilir. Bunun için şema odaklı tedavi yaklaşımlarının olduğu ileriki çalışmalara ihtiyaç duyulmaktadır.
