Cognitive Behavioral Psychotherapy of a Severe Anorexia Nervosa Case

Abstract

Anorexia Nervosa is a chronic, severe psychiatric illness characterized with life threatening weight loss. Patients with eating disorder almost devote their lives to lose weight. In the course of disorder, patients hold irrational fears of becoming overweight and are committed to lose weight with/without engaging bulimic behaviors. The effectiveness of drug treatment and psychotherapy is scant. Therefore, in this paper, treatment process of a 28 year-old patient with anorexia nervosa whom hospitalized to inpatient unit with 33kg is presented to discuss the effectiveness CBT treatment. After two weeks of intense psychiatric care, 10 sessions of CBT is delivered in inpatient unit resulting with significant improvements in her weight control behavior. She completed 6-week inpatient treatment and followed by this, she completed her treatment process as an outpatient client. By the end of treatment, she reached 50 kg and sustained her weight during 6 month follow up period. The records revealed that she manages to cope with her fears of gaining weights and stopped using safety behaviors. Moreover, it is reported that her maintaining behaviors like excessive exercising habits, purging and restrictive dieting abated. Exposure intervention is combined with CBT treatment introduced to help her to first accommodate an imaginary acceptance to idea of gaining weight and tolerating to reach her healthy targeted weight. This followed by using exposure interventions as a preventative instrument to help her overcome her fears of gaining weights in the future and maintain the healthy weight.


Ağır bir Anoreksiyalı Olgunun Bilişsel Davranışçı Psikoterapi Süreci

Anoreksiya nervoza kronik seyirli, kilo kaybı ile hayatı tehdit edebilen ağır bir ruhsal rahatsızlıktır. Hastaların en önemli korkusu kilo almaktır ve kendini kilo vermeye adamıştır. Kilo vermek için sık çıkartma, aşırı egzersiz, yeme kısıtlama davranışları izlenir. Anoreksiyanın tedavisinde ilaç tedavilerinin ve psikoterapilerin etkinliği sınırlıdır. Bu sunumda son 2 yılda 33 kiloya kadar kilo kaybeden ve psikiyatri servisinde yatırılarak tedavi başlanan 28 yaşında ağır bir anoreksiya nervozalı olgu tartışılmıştır. İlk 2 haftalık psikiyatrik bakım ardından, 10 seanslık bir BDT sürecinde başarılı biçimde tedavisi yürütülmüştür. Olgu, 8 haftalık yatarak tedavi gördükten sonra, devam eden terapi sürecini ayaktan tamamlamıştır. Sonuçta 50 kg ulaşmış ve 6 aylık izlem sürecinde kilosunu koruyabilmiştir. Olgu bilişsel davranışçı psikoterapi altında kilo alma korkusunu azaltabilmiş, kusma ve kısıtlama davranışlarını kontrol edebilmiş sonrasında anoreksiyi sürdürücü güvenlik sağlayıcı davranışlarını, aşırı fiziksel egzersizleri ve kusma davranışları azaltmıştır. Azaltılan güvenlik sağlama davranışları arasında kaç kilo olduğunu kontrol etmek, bedenini elleriyle ile kontrol etmek, görünüşünü sormak yer almıştır. Maruz bırakma alıştırmalarından yarar gören olgu normal kilosuna alışabilmiş ve uzun sure etkili biçimde kilo alma korkusunu azaltmayı başarmıştır.
