Applied Muscle Tension and Exposure During the Cognitive Behavioral Therapy of a Adolescent with Blood-Injection-Injury Phobia: A Case Report

Abstract

Blood-injection-injury phobia (BII) is a specific type of phobia that includes anxiety and avoidance behaviors against blood, injection, injuries or medical practices. Children and adolescents are seen with a frequency of 0.8% to 1.5% and the median age of onset is 5.5. It has been shown to have some unique factors (fainting, disgust, physiological symptoms, and pain) compared to other phobia subspecies. The increase in heart rate and blood pressure follows a sudden decrease, and this condition is called vasovagal syncope. Cognitive Behavioural Therapy (CBT) is one of the most effective treatment methods in BII. CBT can be applied for one session or 8–12 sessions. CBT includes psychoeducation, automatical thoughts detection and restructuring, prevention of exposure and avoidance. Blood pressure increasing techniques, especially applied muscle tension technique, to prevent syncope during exposure, increase the effectiveness of therapy. In this case report, the treatment process of adolescent patient admitted with blood-injection-injury phobia successfully completed using CBT techniques will be discussed. After ten sessions of cognitive behavioural therapy, the patient’s blood concerns and avoidance behaviours were completely gone. He managed to give blood, which he stated the highest degree of discomfort in the hierarchy of fear, without experiencing syncope. It has been demonstrated with the fact that CBT, which involves exposure, especially in combination with blood pressure raising techniques, is effective in the treatment of BII in children and adolescents, as in adults.


Kan-İğne-Yaralanma Fobisi Olan Bir Ergenin Bilişsel Davranışçı Terapi Sürecinde Uygulamalı Kas Germe ve Maruz Bırakma: Bir Olgu Sunumu

Kan-iğne-yaralanma fobisi (KİYF) kan, iğne, yaralanma, sakatlanma ya da tıbbi uygulamalara karşı duyulan kaygı ve kaçınma davranışlarını içeren özgül fobi türlerindendir. Çocuk ve ergenlerde %0,8 ila %1,5 sıklıkta görülür ve ortalama başlangıç yaşı 5,5’tir. Diğer fobi alt türleriyle karşılaştırıldığında, bazı benzersiz faktörlere (bayılma, iğrenme, fizyolojik semptomlar ve ağrı) sahip olabileceği gösterilmiştir. Kalp hızı ve kan basıncındaki artışı ani bir düşüş izlemekte ve bu durum vazovagal senkop olarak adlandırılmaktadır. KİYF’de Bilişsel Davranışçı Terapi (BDT) en etkili tedavi yöntemlerindendir. BDT tek seanslık ya da 8–12 seanslık uygulanabilir. BDT; psiko-eğitim, otomatik düşüncelerin tespit edilmesi ve yeniden yapılandırma, maruz bırakma ve kaçınmaların engellenmesini içermektedir. Maruz bırakma sırasında senkopu engellemek için kan basıncı yükseltme teknikleri, özellikle uygulamalı kas germe tekniği terapi etkinliğini arttırmaktadır. Bu olgu sunumunda kan-iğne– yaralanma fobisi ile başvuran ergen hastanın BDT teknikleri kullanılarak başarı ile tamamlanan tedavi sürecinden bahsedilecektir. On seans bilişsel davranışçı terapi sonrası hastanın kan ile ilgili kaygıları ve kaçınma davranışları tamamen geçmiştir. Korku hiyerarşisinde en yüksek rahatsızlık derecesi belirttiği kan vermeyi senkop yaşamadan başarmıştır. Çocuk ve ergenlerde de erişkinlerde uygulanan BDT özellikle kan basıncı yükseltme teknikleriyle birlikte uygulanan maruz bırakmanın KİYF tedavisinde etkili olduğu bu olgu ile gösterilmiştir.
